Insurance Benefits

     Medical
    • The employee is, typically, eligible for coverage beginning the 1st of the month after employment begins
    • Bridgeborn offers both HMO and PPO plans through Anthem BC/BS.
    • Employees must work 30 hours a week or more to be eligible for health insurance.
     Dental
    • The employee is, typically, eligible for coverage beginning the 1st of the month after employment begins.
    • Bridgeborn offers Dental coverage through Assurant Employee Benefits.
    • Bridgeborn will pay 100% of the Dental “Low” Option.
    • The employee has the option to “buy up” to the Dental “High” Option.
    • Employees must work 30 hours a week or more to be eligible for health insurance.
     Life Insurance
    • The company provides life insurance coverage of 1 times annual salary up to a maximum of $100,000.
    • The employee has the option to purchase additional coverage up to 5 times annual salary with a maximum of $100,000.
     Short and Long Term Disability
    • The company provides both short and long term disability coverage.
